Job summary

Kalyra Communities Mt Barker is seeking a CHSP Community Coordinator to coordinate person-centred home support, build relationships and manage care plans and teams to deliver quality ageing-at-home outcomes.

You will work across referrals, inquiries, and care plan development, collaborating with multidisciplinary teams to ensure an exceptional customer experience and continuous service improvement in a values-driven organisation.

Qualifications

• Certificate IV or higher in Aged Care, Community Services, or a related field.
• Senior First Aid Certificate.
• Current SA driver’s license.
• Satisfactory police check/clearance.

Responsibilities

• Build strong relationships with customers, families and stakeholders to support engagement and participation.
• Coordinate home support workers and develop tailored care plans.
• Manage referrals and service offerings to ensure customer needs are met.
• Respond to enquiries and complaints professionally.
• Monitor service delivery and adjust plans to maximise outcomes.
• Maintain accurate client records in CRM systems.
• Collaborate with multidisciplinary teams to support continuity of care.
• Contribute to service growth through community engagement and lead generation.
• Undertake scheduling of CHSP client service rosters when required.
