As a CHSP Community Coordinator you will deliver high-quality, person-centred home support services by understanding and responding to individual customer needs. In this role, you will build strong relationships with customers, families and stakeholders, manage referrals and service offerings, and ensure an exceptional customer experience through proactive communication and problem solving. You will coordinate home support workers and develop tailored care plans that promote independence, wellbeing and active ageing. Working collaboratively with multidisciplinary teams and external partners, you will manage case plans end-to‑end, respond to enquiries and complaints professionally, and contribute to continuous service improvement and growth.
