Qualifikationen

• Tertiary Allied Health qualification or RN qualification.
• Current AHPRA Registration (if applicable).
• Eligibility for membership of Allied Health Professional Association.
• Current Victorian Driver’s Licence.
• Nationally Coordinated Criminal History Check (or willingness to obtain).

Aufgaben

• Conduct holistic clinical and non-clinical assessments of older people in community and hospital settings.
• Review support plans, undertake home risk screening and complete My Aged Care triage as required.
• Involve clients and carers in assessment and care planning, developing short- and long-term plans.
• Participate in team case conferences and implement recommendations to support care.
• Liaise with clients, carers, referrers and providers to coordinate care and optimise outcomes.
