Job summary

BaptistCare NSW/ACT - Aged Care is seeking a Care Facilitator to support clients in the Westlakes area, enabling them to live at home with personalised care plans. The role involves home visits, collaboration with families and a multidisciplinary team to deliver funded services that meet individual needs.

Ideal candidates have a degree or diploma in Community Services, strong communication and time management, and a passion for elder care.

Qualifications

• Bachelor’s Degree or Diploma of Community Services (Service Coordination), or Diploma of Community Services (Case Management), or equivalent knowledge and experience
• Intermediate level of computer skills
• Excellent time management and organisational skills
• Proven experience working within funding constraints and achieving service delivery objectives within government program guidelines
• Experience with aged care or transferrable skills is highly regarded

Responsibilities

• Home visiting clients in the community referred for help at home
• Development, implementation and evaluation of services and programs meeting clients’ individual needs
• Developing relationships with families and carers to acknowledge their needs
• Working with clients to set individual goals that are realistic and achievable
• Implementation of best practice and evidence-based approaches within a multidisciplinary team
• Documentation and use of client record management systems
