PlanCare is seeking an experienced and compassionate Co-Care Partner to coordinate high-quality, person-centred home care services for clients in NSW. This office-based role sits in Parramatta with 9am–5pm hours, and after probation you may work from home one day per week.

You will engage with clients, families and care professionals to ensure responsive, safe, and well-coordinated support, building trusted relationships and taking ownership of the client experience.
