Job summary

The Seniors Assessment Service at Western Sydney Local Health District is seeking a Clinical Aged Care Assessor to conduct comprehensive ACAT assessments and create tailored support plans for older people and their carers. Based at Westmead Hospital, you’ll work within a highly skilled multidisciplinary team across hospital and community settings.

Qualifications

• Postgraduate experience with older people in acute or community settings.
• Ability to work independently and in a multidisciplinary team.
• Understanding or experience in health and functional support needs of older people and carers.

Responsibilities

• Complete holistic aged care assessments considering medical, functional, cognitive and environmental needs.
• Develop evidence‑based support plans and facilitate access to aged care services.
• Collaborate with multidisciplinary colleagues and external providers to coordinate care.
• Maintain accurate clinical documentation and manage assessment data.
• Respond to referrals efficiently while meeting service performance indicators.
