The Seniors Assessment Service at Western Sydney Local Health District is seeking a Clinical Aged Care Assessor to conduct comprehensive ACAT assessments and create tailored support plans for older people and their carers. Based at Westmead Hospital, you’ll work within a highly skilled multidisciplinary team across hospital and community settings.

  • Maintain accurate clinical documentation and manage assessment data.
  • Respond to referrals efficiently while meeting service performance indicators.

Remuneration:$81,748.17 - $114,771.98 per annum + superannuation(Dependant on Qualifications)
Hours Per Week:up to 38 (Job Share may beconsidered for part time hours)

What you'll be doing

As a Clinical Aged Care Assessor (ACAT), you'll conduct comprehensive assessments to determine eligibility for Commonwealth-funded aged care services while developing tailored support plans that meet the individual needs of older people and their carers.

You will be required to:

Complete holistic aged care assessments considering medical, functional, cognitive, psychosocial, cultural and environmental needs.

Develop evidence-based support plans and facilitate access to residential aged care and community services under the Aged Care Act.

Collaborate with multidisciplinary colleagues and external service providers to coordinate seamless, person-centred care.

Maintain accurate clinical documentation and manage assessment data in accordance with legislative and organisational requirements.

Respond to referrals efficiently while achieving service performance indicators and maintaining high-quality assessment standards.

Tertiary qualification & current registration with AHPRA in Nursing, Physiotherapy, Occupational Therapy or tertiary qualification in Social Workwith eligibility for membership of the Australian Association of Social Work or Speech Pathology with eligibility for membership of Speech Pathology Australia.

Selection Criteria

Demonstrated post graduate experience working with older people in an acute or community setting.

Demonstrated ability to work independently & in an effective manner encompassing appropriate initiative, organisational, problem solving & time management skills.

Demonstrated ability to work autonomously as well as function as a member of a multidisciplinary team.

Understanding or experience in the health & functional support needs of older people & their carers in the community setting.

Current Drivers Licence (minimum class C) valid within Australia & demonstrated competent driving ability/ experience.

Westmead hospital car parking allocation model is changing. There is a waitlist for parking for new staff commencing at Westmead Hospital. This does not apply for staff with disability.
