Job summary

APM seeks a Clinical Aged Care Assessor to identify needs of older adults and link them with services supporting independent living and quality of life. You will travel to aged care homes to complete Residential Aged Care Funding Assessments using the AN-ACC tool, ensuring person-centred outcomes for residents and providers.

Role offers autonomy with peer support, paid travel, and fully-paid training with CPD points.

Qualifications

• 5 years’ postgraduate experience in aged care (incl. dementia).
• Tertiary qualif. in Nursing, Physiotherapy or OT with unrestricted AHPRA registration.
• Ability to conduct clinical assessments using various tools.
• Strong organisation and ability to work independently and in a team.

What you bring:

• 5 years’ experience post graduate experience in aged care including working with people with dementia.
• Relevant tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy with unrestricted AHPRA registration.
• Ability to conduct clinical assessments using a range of assessment tools.
• Strong organisational skills and the ability to work independently and as part of a team.
• A genuine desire to improve the lives of older Australians.
• Your own vehicle as travel will be required within your region.
