Job summary

APM is seeking a Clinical Aged Care Assessor to identify needs of older Australians and connect them with services for independent living. You will travel throughout your service area daily to meet client needs and support where it’s needed most.

You will work in a multidisciplinary environment, performing thorough assessments, referrals, and promoting function while engaging families and carers. The role supports compliance with Department of Health contractual requirements.

Qualifications

• Minimum 1 year working with older Australians, including client assessments or service delivery.
• Tertiary health-related degree with professional registration (AHPRA).
• Proficiency in Word, Excel, Outlook and client management systems.
• Current Australian driver’s licence and ability to travel within your service area.

Responsibilities

• Conduct high-quality Aged Care Assessments to identify needs.
• Facilitate referrals to essential services for independent living.
• Promote functionality and daily living capabilities.
• Engage families and carers as key stakeholders; ensure compliance with requirements.
