Job summary
Care Connector (Holistic Nursing Solutions) invites applications for Registered Nurse (RN) and Enrolled Nurse (EN) roles in Brisbane, delivering home and community-based care to participants with complex needs.
You will work under experienced clinicians, provide safe, person-centred care in participants’ homes, coordinate with care plans, and build lasting professional relationships while advancing your specialised nursing skills in wound, continence, and spinal cord injury care.
Qualifications
- Current AHPRA registration.
- A current driver’s licence and reliable vehicle.
- Current National Police Check, or willingness to obtain one.
- Work rights in Australia.
- Experience in community or aged care is highly regarded.
- Experience in complex wound care and continence care is highly regarded.
- Experience with spinal cord injury care is highly regarded.
- Passion for client-centered care and service excellence.
- Excellent clinical judgement and attention to detail.
- Strong communication and a calm, professional approach.
- Flexibility to travel within the local area to support clients at home.
- Availability to start immediately is highly regarded.
Responsibilities
- Deliver safe, person-centred care to participants with complex needs.
- Support high-intensity clinical care (with training and supervision), including: spinal injury care, wound care, continence support and catheter care, medication assistance.
- Contribute to care planning and clinical oversight (RN).
- Support and guide support staff where required (RN).
- Follow care plans and positive behaviour support strategies.
- Monitor and report changes in participant condition.
- Complete documentation (progress notes, incident reports).
- Build strong, professional and respectful relationships with clients.
