Job summary

Care Connector (Holistic Nursing Solutions) in Brisbane seeks Registered Nurses and Enrolled Nurses for community-based opportunities across the city. You will deliver compassionate, person-centred care in participants' homes and local settings, supported by experienced clinicians to manage complex cases.

The role focuses on high-quality clinical care, care planning, wound and continence care, and building long-lasting relationships with clients.

Qualifications

• Current AHPRA registration.
• Current driver’s license and reliable vehicle.
• Current National Police Check, or willingness to obtain one.
• Must have work rights in Australia.
• Previous experience in community or aged care.
• Experience in complex wound care and continence care.
• Experience with spinal cord injury care.
• Genuine passion for client-centered care and service excellence.
• Excellent clinical judgement and attention to detail.
• Strong communication and a calm, professional approach.
• Flexibility to travel within the local area to support clients in their homes.
• Availability to start immediately.

Responsibilities

• Deliver safe, person-centred care to participants with complex needs.

Care Connector (Holistic Nursing Solutions) is a clinician-led provider delivering holistic, person-centred care across the community.

About the Role

We are seeking Registered Nurses (RN) and Enrolled Nurses (EN) for community-based opportunities across Brisbane.

This is not high-volume agency work, you will:

• Work with complex and meaningful cases
• Be supported by experienced clinicians
• Deliver care in participants’ homes and community settings
• Build long-term professional relationships with participants

Key Responsibilities

• Deliver safe, person-centred care to participants with complex needs
• Support high-intensity clinical care (with training and supervision), including spinal injury care, wound care, continence support and catheter care, medication assistance
• Contribute to care planning and clinical oversight (RN)
