Job description

£60,000 per year | Full-time | 25-30 days' annual leave plus bank holidays

A well-established and forward-thinking Primary Care Network (PCN) in Cambridge is looking to recruit an experienced Advanced Nurse Practitioner (ANP) to join its dedicated Care Home Team.

This is an excellent opportunity for an experienced ANP to take on a rewarding role providing high-quality clinical care to care home residents, working as part of a supportive multidisciplinary team. You will be responsible for carrying out care home ward rounds, assessing patients and managing their ongoing clinical needs, with the support of the wider PCN team.

Key Responsibilities

Working autonomously as part of the wider primary care team, you will provide high-quality clinical assessment, diagnosis, treatment and management to patients across the PCN's care homes.

The role will include:

  • Carrying out regular care home ward rounds
  • Undertaking autonomous clinical assessments and patient consultations
  • Diagnosing and managing a range of acute and long-term conditions
  • Developing, implementing and reviewing appropriate treatment plans
  • Prescribing safely and effectively within your scope of practice
  • Providing clinical advice and supporting patients with ongoing health needs
  • Identifying and managing deteriorating patients and escalating care where appropriate
  • Working collaboratively with GPs, Practice Nurses, HCAs, Pharmacists and other members of the multidisciplinary team
  • Supporting hospital admission avoidance and appropriate care planning where required
  • Maintaining high standards of clinical governance and patient safety
  • Contributing to the ongoing development of PCN services
