Practice Plus IUC in Ipswich is seeking an Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join the Suffolk and North East Essex UCCH, coordinating urgent care and avoiding unnecessary hospital admissions. Qualifications

  • Evidence of Level 7 continuing professional development

  • Independent prescribing experience in urgent, primary or community care
  • Strong remote consultation skills and ability to assess and manage patients of all ages independently

Responsibilities

  • Assess and manage patients through telephone and, where required, face-to-face consultations using evidence-based clinical practice

Join Practice Plus IUC as an Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) within the Suffolk and North East Essex Unscheduled Care Coordination Hub (UCCH), an innovative multidisciplinary service focused on improving patient outcomes, avoiding unnecessary hospital admissions and keeping care closer to home. Based in Ipswich, the hub brings together clinicians, paramedics and community partners to support patients with urgent care needs. Working alongside colleagues from urgent care, community, mental health and wider healthcare services, you will use your clinical expertise to assess patients, coordinate appropriate care and help ensure patients receive the right support in the most suitable setting. This role is particularly suited to ANPs and ACPs with experience in primary care and independent prescribing. A key part of the role involves completing remote consultations, reviewing urgent care cases and supporting timely clinical decision-making. You'll receive training to develop your telephone consultation skills and contribute to the Suffolk and North East Essex Clinical Assessment Service when required. As a valued member of a collaborative multidisciplinary team, you'll play an important role in service development, quality improvement and patient safety while representing Practice Plus IUC with professionalism and compassion.

Main duties of the job

Assess and manage patients through telephone and, where required, face-to-face consultations using evidence-based clinical practice. Review lower-acuity cases within ambulance service pathways and identify appropriate alternatives to emergency department attendance. Make timely clinical decisions, provide treatment advice, prescribe remotely and arrange onward referrals where appropriate. Work autonomously within your scope of practice to assess, treat, discharge or refer patients to suitable services. Collaborate with multidisciplinary colleagues and system partners to deliver safe, coordinated patient care. Complete triage activities and support referrals across primary care, community, mental health, pharmacy and secondary care services. Provide safety-netting advice, maintain accurate clinical records and use available patient records to support decision-making. Contribute to service improvement, clinical governance, safeguarding and quality assurance initiatives. Support the Clinical Assessment Service during periods of reduced UCCH workload and act as an ambassador for Practice Plus IUC.

About us

ANP or ACP educated to Master's level (or equivalent) with a non-medical prescribing qualification and evidence of Level 7 continuing professional development. At least six months' experience in primary care, including home visiting, telephone triage and face-to-face consultations. A minimum of six months' independent consulting and prescribing experience within urgent, primary or community care. Strong remote consultation skills and the ability to assess and manage patients of all ages independently. Knowledge of integrated urgent care services, including NHS 111, Clinical Assessment Services and Out of Hours care. Understanding of wider health and social care pathways, safeguarding processes and multidisciplinary working. Excellent communication, interpersonal and prioritisation skills, with the ability to perform effectively under pressure. A commitment to patient safety, high-quality documentation, continuous professional development and collaborative teamworking.
