Job summary

Canberra Health Services (CHS) is recruiting for an Advance Care Planning role at The Canberra Hospital, ACT. This multi-classified position offers RN2, HP3 or ASO6 levels, with a focus on guiding conversations, completing care planning documentation and supporting patient-centred decisions across the care team.

You will deliver training to staff and community groups, drive quality improvement activities, and collaborate with internal and external stakeholders to strengthen Advance Care Planning

Qualifications

• Experience in Advance Care Planning or healthcare settings.
• Ability to guide conversations and complete advance care planning documentation.
• Experience delivering training to staff and community groups.
• Ability to work with internal and external stakeholders to improve service outcomes.
