Qualifications

• Current registration to practice as health professional.
• Eligibility for Working with Vulnerable People background check (ACT).
• Ability to guide sensitive conversations and communicate options clearly.
• Willingness to work across RN2, HP3 or ASO6 classifications.

Responsibilities

• Guide sensitive advance care planning conversations with patients and families.
• Assist with completing advance care planning documentation.
• Provide clear information to help patients understand choices and preserve their voice.
• Support teams to make patient-centred decisions in urgent or complex situations.
• Contribute to training, quality improvement, and stakeholder engagement across ACT.
