Responsibilities

• Provide person-centered planning by engaging client and family to understand goals and routines.
• Deliver complex physical care including mobility, transfers and daily tasks as per plan.
• Support medical needs and liaise with care team for changes in condition.
• Foster social participation and community access aligned with client interests.
• Support autonomy, advocate for client decisions and respect consent.

• You’ll work with people who have big, real world needs, from mobility and medical care to daily routines, communication, and fun activities.

What you’ll do:

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

Who we’re looking for:

• Interest or experience in complex care, disability, and health nuances.
• Great communicator: listens well, reads cues, and adapts to different styles.
• Reliable and professional, with good notetaking and teamwork.
• Flexible and adaptable; comfortable with changing needs and shifts.

• Optional but nice: Manual handling, epilepsy management, or disability-support training.
