Job summary

Yerrabi Yurwang Child & Family Aboriginal Corporation is recruiting an Aboriginal Health Practitioner to enhance clinical services at Yerrabi Health Hub in Canberra. The role focuses on delivering culturally safe primary health care and assisting with Medicare processes under supervision of the Practice Manager.

Successful applicants will need to obtain the compulsory CAEC assessment and meet WWVP, Police check, and driver’s license requirements.

Qualifications

• Approved Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice) or equivalent
• Registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Aboriginal Health Practitioner or eligible
• Experience delivering primary health care or community health services
• Understanding of chronic disease management and clinical follow-up processes
• Demonstrated ability to work with GPs, nurses, and other allied health teams
• Strong organisational skills and cultural engagement abilities
• Knowledge of Medicare, MyHealth Record and Practice Incentives Program (PIP)
• Experience with clinical management systems (preferably Best Practice)
• Experience working in Aboriginal Community Controlled Health sector

Responsibilities

• Deliver culturally safe primary health care including assessments and treatments
• Coordinate recalls, reminders, and follow-up care to improve engagement
• Provide wound care, immunisation support, and health promotion activities
• Work with GPs and nurses to ensure coordinated care and accurate documentation
• Organise and participate in community health promotion activities
• Assist with Medicare billing and Best Practice processes in collaboration with the Practice Manager
• Contribute to quality improvement, audits, and incident reporting
