Job summary

Yerrabi Yurwang is seeking an Aboriginal Youth Care Coordinator to support Aboriginal and Torres Strait Islander children under 16 and their families to access culturally safe, bulk-billed primary health care through the Yerrabi Yurwang Health Hub. The role collaborates with the Registered Nurse, GP services and other programs to identify needs, reduce attendance barriers and coordinate referrals.

Qualifications

• Aboriginal and/or Torres Strait Islander person with strong community ties.
• Certificate IV in Aboriginal/Torres Strait Islander health care or higher.
• Experience with Aboriginal children, families, health or community settings.
• Ability to engage families and provide culturally safe outreach and support.
• Trauma-informed, strengths-based, family-led practice.
• Ability to collaborate with clinical staff and program teams.
• Strong communication, advocacy, organisation and record-keeping skills.
• Current Working with Vulnerable People registration and driver's license.
• Experience in Aboriginal Community Controlled Organisations or Aboriginal Health Service.
• Experience with care coordination, health promotion or brokerage support.

Responsibilities

• Engage Aboriginal children and families with culturally safe outreach and care coordination.
• Identify health priorities and reduce barriers to accessing primary health care.
• Coordinate referrals, recalls and follow-up with families and services.
• Support transport and brokerage for access to care as needed.
• Track referrals and ensure continuity of care and outcomes.
• Maintain client records in line with policy and funding requirements.
• Support participation in health assessments and preventive care.
• Contribute to data collection and program reporting.
