La Vita Bella Home Health and Hospice is seeking compassionate RNs to provide one-on-one home care, building meaningful relationships with patients and families. This role offers autonomy, flexible scheduling, and opportunities for growth within a supportive team.

You will assess, educate, and coordinate care in patients' homes, collaborating with the care team and community providers to enhance quality of life.

Formación

• Active RN license in good standing.
• Experience delivering home health care and patient education.
• Ability to travel to patients' homes and support families.

Responsabilidades

• Assess patient's condition during visits and chart observations.
• Perform vitals review and medication checks.
• Educate patients and families on home health care procedures.
• Coordinate with care team and community providers.
• Recommend devices or tools to improve quality of life.
• Listen and respond to patient concerns or requests.

Responsibilities

• Assess patient's condition during every visit and chart your observations
• Perform evaluation tasks, including vital signs and medication review
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with the patients care team, including community providers.
• Make recommendations for devices or tools to help improve the patient’s quality of life.
• Listen to the patient and respond to concerns or requests

Qualifications

• 18 years of age or older
• Ability to pass a criminal background check
• Valid driver's license and insurance on personal vehicle
• Ability to lift 50 pounds
• A state registered nurse license required
• Compassionate and friendly demeanor
• Willingness to travel to your patients homes
