Job summary

Rehab Without Walls Neuro Rehabilitation seeks a Licensed Clinical Social Worker to support individuals and families navigating neurological conditions in home and community settings. This part-time, PRN role emphasizes real-world psychosocial support and meaningful one-on-one client relationships.

You will join a neuro rehab team that values autonomy, flexible scheduling, and tangible clinical impact, with hourly pay, documentation time, mileage reimbursement, and drive time compensation.

Qualifications

• Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC) required.
• Current Social Work or Marriage and Family Therapist licensure/certification in state(s) of practice.
• A minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or other acute diagnosis.
• Current BLS (Basic Life Support) First Aid and CPR certifications.
• Demonstrates effective and professional interpersonal, verbal, and written communication skills.
• Able to work independently and as part of a team.
• Demonstrates strong organization skills and attention to detail.

Responsibilities

• Conducts comprehensive evaluations, assesses social, emotional and support needs of patients and families, and sets measurable objectives with the rehab team.
• Identifies and assists patients/families to utilize community resources to achieve objectives.
• Provides direct social work intervention and counsels patients and families.
• Regularly re-assesses treatment effectiveness and progress toward goals.
• Communicates patient needs and progress to the treatment team and families.
• Consults with medical providers regarding evaluation, treatment, progress and discharge.
• Observes and reports changes to the Clinical Coordinator, Program Manager, or DCM and treating physicians.
• Maintains and submits evaluation, treatment and billing documentation per regulations.
• Adheres to Code of Conduct, HIPAA and compliance programs.
• Performs other duties as assigned.
