Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold a valid Occupational Therapist license.
• Experience in skilled nursing or post-acute facilities preferred.
• Strong documentation and communication skills.

Responsibilities

• Evaluate residents and determine treatment plans.
• Deliver evidence-based OT interventions to improve ADLs, mobility, cognition, and safety.
