Centene is seeking a field-based Care Coordinator for the Arkansas Total Care plan. You will outreach to members by phone or home visits to discuss care plans, coordinate activities with providers, and support ongoing education and questions.

Requires a high school diploma or GED with 1–2 years of related experience; travel up to 30% in AR; safety-sensitive position with background checks and a driver’s license. Competitive benefits and flexible schedules.

Responsabilidades

• Outreach to members via phone or home visits to discuss care plans and next steps.
• Coordinate care activities with providers, family and members to adapt plans as needed.
• Answer member and provider inquiries regarding care plans.
• Communicate with care managers and practitioners to ensure continuity of care.
• Support documenting member needs and regulatory compliance.
• Provide education on benefits and resources available.
• Perform other duties as assigned.
• Complies with all policies and standards.
