Job summary

Family Medicine Woodlands Clinic in Chillicothe, OH is seeking a Patient Care Coordinator-LPN for a 100% full-time daytime schedule. The role collaborates with providers and the health care team to deliver high-quality care within CPC+ in an ambulatory setting.

Responsibilities include coordinating patient needs with physicians, nurses, MAs, educators, and social workers; creating care plans for high-risk patients; closing care gaps; and ensuring timely post-discharge follow-up per CPC+

Qualifications

• Graduate of an Accredited Program in Practical Nursing.
• LPN-Licensed Practical Nurse in the state of Ohio.

Responsibilities

• Maintain accurate and risk-stratified patient panels for each provider or care team.
• Coordinate in-office care to align patient needs with physicians, nurses, and other team members.
• Identify and close care gaps by scheduling appointments and documenting actionable notes.
