Job summary

Sheikh Shakhbout Medical City (SSMC) seeks a Patient Care Coordinator, a registered nurse responsible for end-to-end care pathway management. You will coordinate appointments, educate patients and families, triage symptoms, and escalate when needed to the multidisciplinary team.

Responsibilities include data collection, plan development, and collaboration with clinicians and support services to improve access and outcomes.

Qualifications

• Bachelor’s degree in Nursing is required; registration and licence to practise as a registered nurse are essential.
• Postgraduate qualification in care coordination or related field is desirable.
• CCM credential or equivalent case-management certification is desirable.

Responsibilities

• Adheres to approved standards of specialist nursing practice and clinical pathway coordination.
• Collects comprehensive data related to referral volumes, appointment access, and time-to-treatment.
• Analyzes assessment data to identify issues and trends affecting pathway performance.
• Develops individualized care plans with patient and multidisciplinary team involvement.
• Implements plans to improve pathway efficiency and service delivery.
• Provides patient and family education and coordinates with support services.
• Escalates deterioration or complex symptoms promptly to the clinical team.
• Evaluates progress and updates plans in collaboration with leadership.
• Maintains governance, evidence-based practice, and quality improvement.
