Responsibilities

• Provide and coordinate patient care, educating patients and families about health conditions.
• Assess patient needs, implement and monitor medical plans with the care team.
• Perform systematic patient assessments and record vital signs.
• Define outcomes and develop care plans with the interprofessional team.
• Administer medications and monitor for side effects and reactions.
• Assist in procedures and support patient admission, transfer, and discharge.
