Qualifications

• Provide care across inpatient, outpatient and long-term settings.
• Collaborate with physicians, social workers and therapists to plan patient care.

Responsibilities

• Assess patient health and administer medications as prescribed.
• Observe and record symptoms or changes in patient condition.
• Develop plans of care and ensure patient rights are respected.
• Coordinate with the healthcare team to plan patient care.
• Provide emotional support to families and supervise nursing staff as needed.
