Cambridge Medical & Rehabilitation Centre L.L.C. in the UAE seeks a capable Case Manager to coordinate client cases, liaise with families and healthcare professionals, and monitor service needs. You will assess patient requirements, design care plans, and ensure timely referrals and documentation.

Coordinate and manage client/patient cases to ensure smooth delivery of services.
Act as the primary point of contact for clients, families, and healthcare professionals.
Conduct assessments and identify client needs, concerns, and service requirements.
Develop, implement, and monitor individualized care or service plans.
Coordinate appointments, referrals, and follow-up services.
Maintain accurate case records, reports, and documentation.
Liaise with multidisciplinary teams to ensure effective communication and continuity of care.
Provide guidance, support, and information to clients and their families.
Monitor case progress and ensure timely resolution of issues and concerns.
Ensure compliance with organizational policies, procedures, and confidentiality requirements.
Prepare case summaries, reports, and updates for management.
Support quality improvement initiatives and service excellence.
Handle client inquiries and resolve concerns professionally and efficiently.
Assist in coordinating community resources and support services when required.
Bachelor's Degree in Healthcare Administration, Nursing, Social Work, Psychology, Public Health, Business Administration, or a related field.
Minimum 2-5 years of experience in case management, patient coordination, healthcare administration, customer service, or a similar role.
Fluent in Arabic and English (written and spoken).
Strong interpersonal, communication, and relationship management skills.
Ability to coordinate effectively with clients, families, healthcare professionals, and external stakeholders.
Excellent organizational and time management skills with the ability to manage multiple cases simultaneously.
Strong problem-solving, decision-making, and conflict-resolution abilities.
Experience in maintaining accurate records, reports, and documentation.
Proficient in Microsoft Office applications (Word, Excel, PowerPoint, Outlook).
Understanding of healthcare processes, patient support services, and care coordination is preferred.
Ability to handle sensitive and confidential information with professionalism.
Customer-focused with a compassionate and service-oriented approach.
Candidates currently available in the UAE are preferred.
