Imagine Pediatrics is a tech-enabled, pediatrician-led medical group reimagining care for children with special health care needs through virtual-first and in-home medical, behavioral, and social care.

We are seeking a licensed social worker to monitor high-risk pediatric patients, conduct biopsychosocial assessments, and develop comprehensive care plans. The role involves crisis intervention, care coordination, and collaboration with schools and family services.

Qualifications

• Master's degree in social work or related field.
• LMSW licensure.
• 3-5 years post-graduate healthcare/social work experience.
• Experience working with pediatric populations and family systems.
• Proficiency in motivational interviewing and assessment techniques.
• Knowledge of crisis prevention and intervention.
• Experience providing telehealth services.
• Proficiency in MS Office Suite and online platforms.
• Knowledge of trauma-informed care practices.
• Ability to work afternoons and evenings.
• Willingness to travel up to 10% for training.

Responsibilities

• Resource external services such as social services agencies and behavioral specialists.
• Provide crisis intervention to de-escalate and stabilize patients and families.
• Offer psychoeducation on mental health diagnoses and treatment adherence.
• Collaborate with external care teams including schools, specialists, and child welfare agencies.
• Participate in interdisciplinary team consultations.
