Dream Medical is working with a modern GP practice in Chorley who are currently looking for a Locum Practice Nurse to join their team. Ideally, the Practice is looking for a full-time locum Practice Nurse to cover maternity leave on a 12 - 18 month contract. You would be required to work Monday - Friday as part of a team alongside GP's, Nurses, and healthcare assistants.

Requirements:

1. Knowledge or experience of Chronic Disease Management
2. The care of patients presenting with undifferentiated health problems
3. Child and adult immunisations
4. Cervical Cytology
5. Diabetes
6. Asthma and COPD monitoring
7. Family planning
8. Ability to work as part of a multidisciplinary team
9. Up to date knowledge of QOF and Enhanced Services
10. Be fully up to date in terms of clinical updates, immunisations, and vaccinations
