Job summary

Placements24 is seeking a dedicated Remote Pharmacist to join our clinical services team, serving patients across Gauteng and beyond. This fully remote role offers flexibility to work from a home office near Soweto, with opportunities to impact health outcomes through telepharmacy.

The ideal candidate holds a Pharm.D. or equivalent, is licensed in South Africa, and has 3–5 years of clinical pharmacy experience. Proficiency with EHR systems and pharmacy software is required.

Qualifications

• Doctor of Pharmacy (Pharm.D.) degree or equivalent.
• Current and unrestricted Pharmacist license in South Africa.
• Minimum of 3-5 years of clinical pharmacy experience, preferably in a community or outpatient setting.
• Proficiency with electronic health record (EHR) systems and pharmacy software.
• Excellent communication, interpersonal, and patient education skills.
• Ability to work independently, manage time effectively, and maintain a high level of productivity in a remote work environment.

Responsibilities

• Conduct comprehensive medication reviews (CMRs) and develop personalized medication action plans.
• Provide patient counseling on medication adherence, potential side effects, and disease management.
• Respond to drug information queries from healthcare professionals and patients.
• Collaborate with physicians and other healthcare providers to optimize drug therapy regimens.
• Ensure compliance with all relevant pharmacy laws, regulations, and company policies.
• Document all clinical interventions and patient interactions accurately in the electronic health record system.
