We are seeking a qualified, goal-driven and self-motivated Dialysis Practitioner to join our multidisciplinary team.
The Dialysis Practitioner delivers safe, high-quality, evidence-based, and person-centred dialysis care in accordance with professional, ethical, and regulatory standards. Working collaboratively within the multidisciplinary team, the practitioner promotes care coordination, shared decision-making, and patient self-management to improve patient safety, clinical outcomes, and the overall patient experience. The role supports clinical governance, quality improvement, and continuous learning by adhering to organisational policies, participating in clinical audits and quality improvement initiatives, monitoring clinical and patient-reported outcomes, and applying evidence-based practice to enhance the quality, safety, and effectiveness of care across the patient journey.
