Job summary

Placements24 is seeking a compassionate Disability Support Practitioner to provide essential care and assistance to individuals with disabilities in Port Elizabeth. You will support clients to live independently, with dignity and community participation.

Responsibilities include personal care, mobility assistance, and promoting activities; you will work with families, monitor progress, and report in line with care plans.

Qualifications

• Experience in disability support or related field preferred.
• Strong understanding of person-centered care principles.
• Excellent interpersonal and communication skills, with empathy.
• Willingness to work flexibly including evenings, weekends, and public holidays.
• Valid driver's license and reliable transport for Port Elizabeth client support.

Responsibilities

• Provide direct support per client plans.
• Assist with personal care, mobility, household tasks, and community access.
• Promote independence and participation in social, recreational, and vocational activities.
