Job summary

WhatJobs Direct is seeking a compassionate Community Support Facilitator for our client in Upington, Northern Cape. This in-person role involves home visits to assist clients with daily living, access services, and social engagement, promoting independence, dignity, and wellbeing.

You will develop personalized support plans with clients and supervisors, monitor progress, and coordinate with the team. A caregiving qualification is preferred, with strong communication, empathy and reliability as

Qualifications

• Relevant caregiving qualification or certification preferred.
• Experience in community services or aged care preferred.
• Excellent interpersonal and communication skills with diverse individuals.
• Empathy, patience and non-judgmental attitude required.
• Ability to work independently and manage time in community settings.

Responsibilities

• Assist clients with daily living activities (personal care, meals, household tasks).
• Provide companionship and emotional support to reduce isolation.
• Facilitate access to community resources, appointments and social activities.
• Develop and implement individualized support plans with clients and supervisors.
• Monitor well-being and report changes or concerns to supervisors.
• Adhere to safety protocols to ensure client and personal safety.
• Maintain accurate, confidential records of client interactions and progress.
• Transport clients to appointments or community outings as needed.
• Encourage client independence and self-advocacy.
• Participate in team meetings and ongoing training sessions.
