Responsibilities

• Conduct home visits to assess family dynamics, needs and safety concerns.
• Develop and implement individualized family support plans with families and stakeholders.
• Provide guidance on parenting, child development, and household management.
• Connect families with community resources including education, health, and financial services.
• Offer emotional support and counseling to family members.
• Monitor progress and document interventions and outcomes.
