Placements24 in Vereeniging is seeking a compassionate Disability Support Facilitator to empower individuals with disabilities to live independently. You will work directly with clients to assess needs, develop personalized plans, and connect them with essential services within the community.

Qualifications

• Experience in disability support or community services.
• Strong understanding of various disabilities and person-centred approaches.
• Excellent communication and empathy.
• Ability to develop and maintain professional relationships.
• Valid driver's license and willingness to travel within the Vereeniging area.
• Commitment to promoting independence and inclusion.

Responsibilities

• Conduct comprehensive assessments of clients' support needs and goals.
• Develop individualized support plans in collaboration with clients and their families.
• Provide direct support, assistance, and encouragement to clients in daily living activities.
• Coordinate services with healthcare providers, therapists, and other support professionals.
• Advocate for clients' rights and ensure access to community resources and opportunities.
• Monitor client progress and adjust support plans as needed.
