About the Role

Our client is seeking a dedicated and skilled Community Health Nurse to join their team serving the Klerksdorp, North West region. This essential role focuses on promoting health and preventing illness within the community through direct patient care, education, and outreach initiatives. You will work with individuals, families, and groups to address health concerns, manage chronic conditions, and improve overall well‑well‑ .

Key Responsibilities

• Provide direct nursing care, including health assessments, screenings, immunizations, and wound care, in community settings.
• Develop and implement health education programs on topics such as chronic disease management, nutrition, hygiene, and preventive care.
• Conduct home visits to assess patient needs, provide care, and monitor health status.
• Educate patients and families on managing chronic conditions like diabetes, hypertension, and HIV/AIDS.
• Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care.
• Participate in community outreach events, health fairs, and screening initiatives.
• Advocate for patients' health needs and connect them with appropriate community resources.
• Maintain accurate and confidential patient records and documentation.
• Stay updated on public health issues, trends, and best practices relevant to the North West region.
• Respond to public health emergencies as needed.

Requirements

• Bachelor of Science in Nursing (BSN) or equivalent qualification.
• Current registration with the South African Nursing Council (SANC).
