Qualifications

Responsibilities

• Conduct outreach within the community to identify individuals needing health support and education.
• Assist community members in understanding and accessing local healthcare services, clinics, and hospitals.
• Provide health education on disease prevention, nutrition, and maternal health.
• Help clients navigate the healthcare system, including appointment scheduling and understanding medical information.
• Facilitate referrals to specialists, diagnostic services, and social support programs.
• Build relationships with healthcare providers and community leaders in Polokwane.
• Collect data on community health needs and service utilisation.
• Promote participation in health screenings and wellness programs.
