Job summary

Placements24 is seeking a dedicated Community Health Navigator to join our Randburg team, operating in a flexible hybrid model. You will bridge individuals with the healthcare system, help them access services, understand conditions, and navigate pathways to care.

You will combine remote support with in-person interactions, coordinate with providers and community resources, and track progress toward health goals, contributing to better health outcomes and equity.

Qualifications

• Bachelor's degree in Health Sciences, Public Health, Social Work, or related field.
• Experience in healthcare, public health, or community services.
• Strong knowledge of the local healthcare landscape and available resources.

Responsibilities
