Job summary

Placements24 in South Africa is seeking a Community Health Facilitator for Klerksdorp, North West. The role focuses on bridging healthcare services with the community, promoting health education, disease prevention, and access to essential care.

You will conduct needs assessments, develop health education programs, facilitate screenings, provide counseling, and collaborate with clinics, NGOs, and community leaders to improve local health outcomes.

Qualifications

• Qualification in Community Health, Public Health, Nursing, or related field.
• Proven experience in community health work or health education.
• Strong knowledge of health issues and South African healthcare system.
• Excellent communication, presentation, and interpersonal skills.
• Ability to travel within the Klerksdorp area.

Responsibilities

• Conduct health needs assessments in the community.
• Develop and deliver health education programs and workshops on nutrition, hygiene, chronic disease management, and maternal health.
• Promote awareness of available health services and assist access to clinics, hospitals, and specialized care.
• Facilitate health screenings and basic health checks.
• Provide counseling and support to individuals and families regarding health concerns.
• Collaborate with local healthcare providers, community leaders, and NGOs to coordinate health initiatives.
• Collect and record health data, maintain accurate client records and report on program activities.
• Advocate for community health needs and address barriers to healthcare access.
• Organize and participate in community health campaigns and outreach events.
