Job summary

Qualifications

• Minimum 2 years in community health or public health role.
• Qualification in Community Health Work or related discipline.
• Excellent communication and interpersonal skills.
• Ability to work with diverse populations in Gauteng.

Responsibilities

• Conduct health screenings and assessments in community settings.
• Provide health education and information on nutrition, hygiene, family planning and chronic disease management.
• Assist individuals in navigating the healthcare system and accessing services.
• Facilitate referrals to doctors, clinics and other providers.
• Support patients in adhering to treatment plans and medications.
• Collect data and maintain accurate records of interactions and health status.
• Organize and lead community health workshops and awareness campaigns.
• Liaise with local clinics, hospitals and community organisations to coordinate care.
• Advocate for community health needs.
