Job summary

Intercare Sandton Physical Rehabilitation, a unit within the Intercare Group, seeks a Case Manager to coordinate patient care between the multidisciplinary team and funders from pre-admission through discharge. You will contact patients and families, ensure efficient processes, coordinate with funders and doctors, participate in meetings, and support discharge planning and reporting.

Qualifications

• Registered with SANC as a Professional Nurse or Enrolled Nurse.
• At least 4 years in a hospital or medical scheme environment; 2-3 years case management recommended.
• Strong understanding of operations, funder rules, and case coordination; basic knowledge of risk management and billing systems.

Responsibilities

• Contact patients and families before admission and at key stages to explain funder requirements, length of stay, authorisation, and discharge planning.
• Ensure that patient processes are efficient and promote satisfaction throughout their stay.
• Coordinate with funders, referring doctors, and other parties regarding patient transfer, authorisation, and benefit management.
• Participate in family and multi-disciplinary team meetings to provide clinical updates and assist with authorisation and care coordination.
• Perform thorough pre-admission assessments to ensure patients meet criteria and that admissions are appropriate.
• Assist with procurement of discharge equipment, oxygen, and provide detailed discharge reports to doctors and funders.
• Monitor and manage the patient's entire case from admission to discharge, ensuring all information is updated for admin staff and funders.
• Build and maintain productive relationships with doctors, funders, and other stakeholders through regular communication and feedback.
• Monitor occupancy levels, identify risks such as infection control concerns, and work toward achieving budgetary and operational targets.
• Create awareness of the facility among key stakeholders and showcase the hospital when needed to encourage referrals and admissions.

REQUIREMENT

• Registered with SANC as a Professional Nurse / Enrolled Nurse
• Atleast 4 Years in a hospital or medical scheme environment; 2 - 3 years case management experience recommended.
• High-level understanding of operations, funder rules, and case coordination; medium knowledge of risk management, billing systems, information systems, and marketing.
• Strong rule-application, attention to detail, intermediate proficiency with words, numbers, and memory.
