Job summary

Wayside Youth & Family Support Network in Framingham/Waltham is seeking a Bachelor’s Level Intensive Care Coordinator to connect with youth in behavioral health services. The role supports a team-focused culture and provides opportunities to positively impact youth under 21.

The coordinator plans and coordinates services, facilitates planning meetings, mentors caregivers in advocacy, and navigates school and state agency resources. Framingham/Waltham site.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field.
• At least 1 year of direct experience with children, young adults, and/or families.
• Valid driver’s license and access to a reliable vehicle.

Responsibilities

• Provide care planning and coordination of services for eligible youth under 21 using the Wraparound model.
• Facilitate Care Planning Team meetings and coordinate multiple services.
• Help caregivers learn effective advocacy skills.
• Collaborate with all team members to ensure continuity in treatment and messages.
• Attend meetings outside of Care Plan Meetings per parent/guardian/youth request.
• Assist in navigating schools, courts, and state agencies (DCF, DMH, DDS) and other community resources.
