Job summary

Wayside Youth & Family Support Network is hiring an Intensive Care Coordinator at our Charlton site. You will provide care planning and coordination for youth under 21 using Wraparound and System of Care principles, partnering with families and schools to coordinate services.

You will lead Care Planning Team meetings, connect families with community supports, and help caregivers develop advocacy skills, while ensuring consistent treatment messages across the team.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, or related field.
• At least 1 year of direct experience with children, young adults, and/or families.
• Valid driver’s license and access to reliable vehicle.

Responsibilities

• Provide care planning and coordination of services for eligible youth under 21 using the Wraparound model.
• Facilitate Care Planning Team meetings and coordinate multiple services/supports.
• Help caregivers learn effective advocacy skills.
• Collaborate with team to ensure continuity in treatment and messages.
• Attend meetings outside Care Plan Meetings as requested by parent/guardian and youth.
• Navigate systems (school, court, state agencies, community resources) for youth and families.
