Job summary

YES House is seeking a TLP Coordinator to lead transitional living services for at-risk youth, ensuring grant compliance and impactful outcomes. You will coordinate housing, education, vocational support and life-skill development through a trauma-informed, person-centered approach.

The role requires supervisory experience, budgeting oversight, and strong case management skills to drive program success and measurable results for youth transitioning to independence.

Qualifications

• Strong case management knowledge: assessment, goal planning, documentation, follow-up.
• Trauma-informed care framework to support positive youth development.
• Ability to interpret grant guidelines, program policies, and applicable laws.
• Highly organized; able to manage multiple projects and deadlines.
• Excellent communication with internal teams, partners, and diverse populations.

Responsibilities

• Mentor and guide youth toward education, employment and access to services.
• Coordinate groups and one-on-one sessions focusing on life skills, employment and education.
• Link Case Managers with WIOA programs, internships, or job coaches for hands-on experience.
• Support tutoring and education, collaborate with local schools to help obtain diplomas or GEDs.
