Job summary

We are seeking a Youth Transition Advocate at YSS in Des Moines, IA to support young people as they prepare for adulthood. The role blends advocacy, case management, and meeting facilitation to empower youth ages 17–23 in their transition process.

Responsibilities include building trust, coordinating referrals, and ensuring safety and access to needed services while maintaining thorough documentation and collaboration with families, schools, and community partners.

Qualifications

• Bachelor's degree or equivalent in social work, human services, case management, nursing, or related field.
• At least 1 year of experience in the human services field.
• Able to drive a YSS vehicle with a valid license and clean driving record.

Responsibilities

• Build trusting relationships to help youth develop confidence, independence, and self-sufficiency.
• Provide individualized case management and Aftercare Services for youth ages 17–23.
• Meet regularly with youth in homes, schools, communities, or virtually.
• Connect youth with resources to reach goals.
• Assist with immediate safety and basic needs; transport to meetings and activities as needed.
• Coordinate steps of the transition process from referral through follow-up.
• Facilitate youth-centered meetings where every voice is heard.
