Job summary

Eckerd Connects in Fort Pierce, FL seeks an Aftercare Resource Navigator Supervisor DJJ to manage referrals, case assignments, and discharge approvals for their program.

You will oversee Navigator teams, ensure participation in Transition Conferences, coordinate with JPO, and help develop YES plans for youth on CR or PCP.

Applicants must have a degree as specified, supervisory experience, and pass background checks; the role offers comprehensive benefits and a path to impact youth reentry.

Qualifications

• Bachelor's degree or Associate degree with two years’ adolescent-focused experience required.
• If using Associate degree, must have two years of relevant experience.
• 1 year supervisory or leadership experience required.
• Applicants must pass drug screen and have valid driver's license.

Responsibilities

• Maintain program referrals, case assignments, and discharge approvals for assigned navigators.
• Ensure navigators participate in Transition Conference and related meetings; assist with Transition Plan.
• Collaborate with JPO to identify resources and support Youth Empowered Success (YES) plan.
• Monitor service initiation within 3 working days after release and conduct reviews in JJIS.
• Maintain regular contact with youth, families, and service providers to track progress.
• Build relationships with DJJ staff, law enforcement, courts, and public defender offices.
