Job summary

Family Support Services in Largo, FL is seeking a Youth Transition and Facilitation Specialist to support the Independent Living team. You will collaborate with foster parents, CMOs, and community providers to ensure youth age 13-17 receive life skills assessments and training, and connect them with mentoring and post-18 program options.

Qualifications

• Bachelor's degree required.
• Minimum 3 years’ experience in community engagement, program management or related fields.
• Experience with youth/teen engagement and child welfare preferred.

Responsibilities

• Coordinate life skills programs and assessments for teens.
• Plan and attend transition staffings for 16–17-year-olds.
• Collaborate with CMOs and foster care providers to meet contractual measures.
• Develop and maintain mentoring relationships and activities.
• Utilize FSFN data to track progress and outcomes.
