Job summary

Family Support Services of Suncoast LLC in Largo, FL is seeking a Youth Transition and Facilitation Specialist to support the Independent Living team by coordinating life skills assessments, connecting youth to mentors, and facilitating transition staffing for 16- to 17-year-olds.

The role partners with foster parents, CMOs, and community providers to prepare teens for post‑18 options, ensuring documentation, and program participation while aligning with state requirements.

Qualifications

  • Education: Bachelors degree required.
  • Experience: Minimum 3 years’ experience in community engagement, program management, volunteer management or event coordination, or youth/teen engagement.
  • Child welfare adoption experience preferred.
  • Clear a Level II Background screening (Live Scan) and a reference check.
  • Clear a local background check from the county in which you reside.
  • Clear a substance abuse screen.
  • Clear a Motor Vehicles Records check and be insurable under the company’s automobile policy; provide proof of personal auto liability insurance.
  • Provide copy of all certifications and degrees prior to first day of employment.
  • Clear an E‑Verify check to substantiate eligibility to work in the United States.

Responsibilities

  • Coordinate with contracted provider to facilitate the Statewide Life Skills Program, life skills assessments and informal training, and manage tracking to ensure participation.
  • Collaborate with community partners to provide teen enrichment programs, evaluate effectiveness, manage budgets and ROI, and secure volunteers as needed.
  • Utilize FSFN data to identify teens requiring transitional staffing, schedule and facilitate transition staffing for 16‑ and 17‑year‑olds, and complete the My Pathways to Success Plan in FSFN.
  • Prepare case information for teens transitioning to secondary case management for 17.5 year‑old staffing and Extended Foster Care preparation.
  • Vet, establish, and maintain relationships with mentoring providers, ensure case‑management partners are aware of opportunities, facilitate utilization of mentoring programs, and connect interested individuals with appropriate partners.

Job Summary

This role supports the Independent Living team by collaborating with foster parents, group home providers, foster care services providers (CMOs), and other community partners to prepare FSS’s youth (13‑17 years old) for transition out of the system of care. Responsibilities include coordinating Life Skills assessments and training, connecting youth to mentors, scheduling and facilitating transition staffing for 16‑ and 17‑year‑olds, ensuring contractual requirements for the 17.5 Judicial Review are met, and helping eligible youth register for the Keys to Independence Program and determine post‑18 options.

Qualifications

Minimum Qualifications:

  • Education: Bachelors degree required.
  • Experience: Minimum 3 years’ experience in community engagement, program management, volunteer management or event coordination, or youth/teen engagement. Child welfare adoption experience preferred.

Licensure, Certification, and/or Registration
