Job summary

LifeWorks in Austin, TX, is seeking a full-time Emergency Shelter Case Manager to support youth ages 13-20. You will provide community-based case management, develop service plans, coordinate with partners, and help clients access housing, benefits, and essential services.

This role emphasizes trauma-informed, strengths-based practice, crisis support, robust documentation, and collaboration with DFPS and local agencies. The position offers competitive pay and comprehensive benefits.

Qualifications

• Bachelor’s Degree in social or behavior sciences or administrative field.
• Minimum 2 years related experience; or equivalent.
• Lived experience of homelessness is a plus and may count toward qualifications.
• Proficient in HMIS, Microsoft Office, Outlook, and agency database.

Responsibilities

• Provide client-centered, healing-centered case management to diverse individuals who may have experienced homelessness.
• Identify and leverage clients’ strengths to help achieve self-defined goals.
• Work with teammates to ensure fidelity of the Strengths Model in documentation and engagement.
• Assist clients with day-to-day needs and development of life skills.
• Assess clients’ wellness and tailor wraparound supports and referrals.
• Manage incidents and crises with de-escalation and safety planning.
• Engage with the community and collaborate with partners to advocate needs and coordinate services.
• Assist clients in building a network of support and relationships.
• Maintain accurate client records, service plans, and progress notes.
