Job summary

ViaQuest Healthcare Central in Columbus, OH is seeking a full-time Incident Reporting coordinator to ensure regulatory compliance and support residents in our intermediate care facilities. You will maintain knowledge of state and federal rules and coordinate with staff to complete assessments and plans.

The role requires a four-year degree in social services, strong communication, and supervisory experience.

Qualifications

• Four-year degree in social services or related field.
• One year of experience in the field of developmental disabilities may substitute for experience.
• Strong customer service and communication skills.
• Supervisor or management experience.
• Displays strong communication skills and possess excellent decision-making and time-management skills.

Responsibilities

• Maintain thorough knowledge of state and federal rules for ICF/IID operation.
• Monitor medication and medical supply management by staff as appropriate.
• Monitor appointments with outside providers as appropriate.
• Complete Individual Assessment forms quarterly with supporting docs.
• Conduct annual and special Individual Plan meetings for residents.
• Update floor book information reflecting current status of individuals.
• Prepare all required plans, assessments, reviews, and meeting notes for individuals served.
• Coordinate utilization of community services based on residents' preferences and needs.

At ViaQuest Healthcare Central we provide quality support and services to individuals with developmental disabilities in our intermediate care facilities (ICF).

Responsibilities may include:

• Maintain a thorough, current knowledge of all state and federal rules, laws, and regulations pertaining to operation of an ICF/IID.
• As appropriate, monitor the management of medications and medical supplies by the employees.
• As appropriate, monitor the management of appointments with outside providers.
• Accurately complete Individual Assessment forms quarterly and maintains necessary supporting documentation.
• Conduct annual and special Individual Plan meetings for each resident on their caseload.
• Complete and update floor book information to reflect the current status of each individual.
• Complete all Individual Plans, Comprehensive Functional Assessments, Quarterly Reviews and Special Team Meeting minutes for individuals served.
• Coordinate and ensure utilization of community services for individuals based on their preferences and needs.
